Payment Information
Candidates from Utah who are currently enrolled in an Educator Preparation Program will receive a test voucher from their program. The voucher must be entered during the registration process. Vouchers cannot be applied retroactively to existing registrations, so be sure to acquire your voucher before you register. If you have any questions about your voucher, ask your faculty advisor before registering for the test. Note that test vouchers may be applied to a Foundations of Reading test registration only and have no cash value.
Test registration fees must be paid using a credit card (VISA or MasterCard only) or a debit or check card that carries the VISA or MasterCard logo and that can be used without the entry of a personal identification number (PIN). Bank cards without a VISA or MasterCard logo cannot be accepted. The merchant name that will appear on your credit card statement is "Eval Systems Test Fee."
If you have a voucher, you may apply your voucher during the registration process.
Your registration is valid for one year from the date of issue. If you do not schedule a test appointment and take your test within one year, or withdraw your registration within one year, your registration will expire and you will receive no refund or credit of any kind.
Examinees without a credit card. If you do not have a VISA or MasterCard, contact Evaluation Systems for information about how to submit payment by check.
